Who can submit the application for issuing the European Health Insurance Card?
The application must be submitted by the direct beneficiary of the European Health Insurance Card.
In case the beneficiary cannot be present, the authorized person will need to additionally submit the following documents: A copy of the identity card and a declaration of own responsibility – if they are a first-degree relative (parent/child) of the beneficiary or a Notarial power of attorney – if they are not a first-degree relative of the beneficiary.
